Grade 6-11 Students Required to Complete Physical Before First Day of School

All incoming grade 6-11 students for the 2012-2013 academic year are required to have a completed physical before the first day of school. We are pleased to offer current and future students the opportunity to complete their physicals here on campus. Doctors will be performing physicals on campus in Room 653 in the Pasternack Field House on:

Wednesday, May 16 from 12 – 3 p.m.


Thursday, May 17 from 11 a.m. – 3 p.m.

The physical examinations will be conducted in compliance with California Interscholastic Federation policy by doctors who are board certified physicians. The cost is $35.00 per student with the physicians donating a portion of this fee to St. Margaret’s.

If you would like your child to undergo this on campus physical, please complete the health history portion of the document and sign the parent consent line. Please have your child bring the physical form with them on the day of the physical. Do not send a check with your form, you will be billed by the Business Office. Due to the large number of students, physicals will be performed on a first come, first serve basis.

If you would rather have your child see your personal physician for this required physical examination, please have the physician complete the same physical form and then submit it to the Athletics Department by July 1, 2012, but not before May 16. Please know that students may not attend school until their approved forms are on file with the Athletics Department.
